Forms

The following forms are for Open University students only. All forms for Cal Poly matriculated students can be found at the Office of the Registrar.

Form	Description
AUDIT COURSE (Powerform)	Use this form if you would like to petition to audit an Open University program.
CREDIT/NO CREDIT GRADING OPTION (Powerform)	Use this form to request that a course be graded on a "credit/no credit" basis.
EXCEED REPEAT LIMITATION (Powerform)	Use this form if you need you would like to petition to exceed the course repeat limitation.
REFUND REQUEST (Powerform)	Use this form to request a refund of Open University course fees. This form cannot be used to request housing or dining refunds.
PETITION TO WITHDRAW FROM OPEN UNIVERSITY COURSE (Powerform)	Use this form if you need to withdraw from the Open University course.
